Sports Physicals

All students in the 5th-12^th  grade who plan to participate in any sport or 7-12^th Band during the 2024-25 school year must have a preparticipation physical before competition begins.  The physical may be done by your primary care physician on the approved OSSAA form or will be offered to all students at Laverne Family Health Clinic on July 27th from 8:00 am to 1:00 pm FREE of charge.  Physicals for all boys and girls will be available free on this one day only.  The OSSAA approved form can be picked up in the high school office, can be obtained at the clinic. or can be found on the OSSAA website at:

The form must be signed by the student and parent prior to physical examination.  The Laverne Family Clinic will collect and bring us the forms, but for others we will need these returned to the HS Office by August 1^st.  If you have questions, you may call the school or Laverne Family Health Clinic.  A huge thank you to Laverne Family Health Clinic for making this available to Laverne students!
